Patient Participation Group

We are looking for patients to join our new patient participation group, we would like to hear from patients who want to be involved, please click on the Patient Group link opposite for further information.

Car Park - again !

Again, patients and visitors to the surgery are not being thoughtful in their use of the carpark. We all realise that space is limited, but please make sure that you leave the surgery carpark as soon as your business at the surgery finishes, and please do not leave your car there whilst you pop over to the Post Office or Lloyds chemist. Also, the area behind the back door and the driveway MUST be kept clear at ALL times for ambulances and other emergency vehicles. Absolutely NO blocking or dropping off in that area, or leaving your car there to pick up a script from the dispensary. This will help everyone who needs to visit the surgery. Thankyou.

FLU INJECTION

If you have NOT had your flu vaccination yet please contact reception and make an appointment with one of the practice nurses.

HOW TO REGISTER AS A PATIENT

steth_book1Either bring your medical card to reception or ask for a registration form.

Alternatively, download a registration form here

Once you have downloaded the form, print it out, complete it and bring it to reception.

Once you have registered, your records from your previous surgery will be sent for, but it can take several months to get these.  Therefore, it is best to register as soon as you can.  When you attend here for the first time, it is helpful to bring your repeat prescription slip with you if you are on regular medication.

You can see whichever doctor you want, but we encourage you to see the same doctor whenever possible, as you will get better continuity of care.  When your doctor is on holiday, you can see another one if your problem really cannot wait.

As a new patient you can request an appointment for a new patient check if you feel it would be appropriate.
